Pfizer acquired it in 2015, and the business has been Hospira, a Pfizer company, since. That is the whole of the relationship, and it is why "pfizer bacteriostatic water" and "hospira bacteriostatic water" are usually two names for the same vial.

The Abbott lineage also explains something useful. An NDC, the National Drug Code identifying a US drug product, begins with a labeler code assigned to the responsible company. Hospira's is 0409, inherited from Abbott, so a genuine Hospira vial carries an NDC starting 0409. That one detail does more counterfeit screening work than anything else on the label.

The product is plain: Sterile Water for Injection with benzyl alcohol 0.9 percent, 9 mg per mL, added as a bacteriostatic preservative. No sodium chloride, no buffer, no tonicity agent, no stabiliser. Hospira supplies bacteriostatic water in multiple-dose fliptop vials in two sizes, 10 mL and 30 mL. The solution is identical. The 30 mL is considerably cheaper per millilitre everywhere it is sold, which makes it look like the obvious buy and is the most common purchasing mistake on this product.

Start with the arithmetic that ignores time. The number of vials a bottle fills is the bottle volume divided by the volume you add per vial.

Now put the clock back in. Those counts are theoretical capacity, and the real limit is whichever runs out first, the volume or the 28 days. Someone working through one 10 mg vial at a time, reconstituting each with 2 mL, uses roughly 2 mL a month. A 30 mL bottle would take over a year to empty at that rate, and about 28 mL of it would go into a sharps bin four weeks after purchase. The 10 mL is the right buy there, and even that has waste in it.

The 30 mL earns its place in three cases: several vials open in the same four week window, which is what a multi-compound protocol looks like; large vials taking 3 to 5 mL each; or more than one person drawing from the same bottle. Outside those, the per millilitre saving is not a saving, because you do not keep the part you throw away.

How to Read a Hospira Bacteriostatic Water Label

Every line on the vial is load bearing, and most people never read past the first one. Working down it in order.

Bacteriostatic Water for Injection, USP

The USP suffix is the important half. It means the product is made to the United States Pharmacopeia monograph of that name, which fixes composition, purity limits and testing. That is what separates a registered drug product from a clear liquid someone filtered into a vial. A label reading "bacteriostatic water" with no USP designation is telling you something.

Benzyl Alcohol 0.9% added as preservative

The only ingredient besides water, at 9 mg per mL. Bacteriostatic means growth suppressing, not killing: it holds the small population introduced by an imperfectly swabbed stopper in check so it cannot multiply between punctures. It does not sterilise the contents and will not rescue a substantially contaminated vial. Multiple-dose

The hook the in-use limit hangs off. A multiple-dose container is one the labeling contemplates being entered more than once, which is only possible because of the preservative. Sterile Water for Injection comes in single-dose containers with no preservative and is discarded after one entry regardless of what is left.

The NDC

Three hyphenated segments: labeler, product, package. The labeler code identifies the company and Hospira's is 0409. The package code identifies the container, which is why the 10 mL and the 30 mL carry different NDCs though the liquid is the same. Two checks: it should begin 0409 on a Hospira vial, and it should match the carton.

Lot number, expiry date and Rx only

Lot and expiry appear on both the vial and the carton, and they should agree. The expiry is the unopened shelf life, typically two to three years from manufacture, and has nothing to do with the 28 day figure covered next. A vial with no lot number is not a pharmaceutical product, whatever else the label says. The 28 Day Rule After First Puncture

Twenty-eight days from the first puncture is the in-use limit, and it is the number people get wrong most often, usually by starting the count from the wrong event.

It is not the water expiring. An unopened vial keeps to the printed expiry date. The 28 days is a limit on an entered multiple-dose container, and two things sit behind it. Preservative efficacy: benzyl alcohol at 0.9 percent is tested against the USP antimicrobial effectiveness standard, and the in-use limit says how long that suppression can be relied on under repeated puncture. And the compounding standard, USP General Chapter 797, which assigns an entered multiple-dose container a 28 day beyond-use date unless the manufacturer specifies otherwise.

Worth being precise about where it is written, because a lot of pages on this subject are not. The vial carries the multiple-dose designation and the package insert carries the discard direction. The 28 days is the figure the standard applies to that designation, not a number stamped on the front of the vial.

The practical version is one sentence: write the date on the label with a marker the moment the needle first goes through the stopper. Not the purchase date, not the date you opened the carton. Every clock in the box runs from a different event and this is the one people conflate.

Day 29 is not a cliff. A carefully handled vial is not dangerous on day 31. The problem is that a contaminated vial and a clean one look identical, no home test distinguishes them, and the cost of respecting the date is the price of a replacement. Storage in the meantime is undemanding: controlled room temperature, in its carton, out of direct light. Refrigeration is not required and does not extend the 28 days. Hospira is the most recognised name on this product but not the only manufacturer. That last cell answers a question that gets asked constantly. Reconstitution solution is not a defined thing. Some bottles carrying the name are bacteriostatic water relabelled, and some say so outright in the product title. Others are proprietary mixes adding sodium chloride or other excipients. The name tells you nothing; the ingredient line tells you everything. What has been destroyed is what the packaging carried: the sterility guarantee of an unopened container, and the date. The 28 day clock started when the repackager punctured the original, on a date you cannot discover, and a multi-dose container whose in-use date you cannot establish cannot be used as one. The other quiet failure is plain sterile water relabelled as bacteriostatic.
