The term has no formal definition here, and it gets used for at least three different things.

Three motives account for nearly all of it, and they are not equally sound.
Side effects. Tirzepatide's gastrointestinal effects are dose related and adapt with exposure, which is why the approved schedule steps up over months rather than starting at target. Two readings of that table are both correct and point in opposite directions. The curve flattens: going from 10 mg to 15 mg buys 1.4 percentage points, while the first 5 mg buys almost twelve. If most of the effect arrives early in the dose range, then extending the curve downward makes a low dose look more interesting than a naive linear reading would suggest.
The other reading is that the lowest point measured is 5 mg, and a curve says nothing about the region past its endpoint. Dose response relationships in receptor pharmacology are sigmoid, not linear, and the bottom of a sigmoid is flat. There is a dose below which a GLP-1 and GIP receptor agonist produces no meaningful appetite effect at all. The Reconstitution Maths
Small doses are where arithmetic errors become expensive, because the volumes involved get close to the resolution of the syringe. The three steps do not change. The top row is the trap. At 10 mg/mL a 0.5 mg dose is five marks on a U-100 barrel, and a one mark misread is a twenty percent dose error. Diluting further moves the same dose into a range where the same absolute misread costs far less proportionally, which is the single most useful adjustment anyone microdosing can make. Two further points on small volumes. Insulin syringes are calibrated for accuracy across their working range, not at the very bottom of the barrel, so a 0.05 mL draw carries more relative error than a 0.4 mL draw. The Compounded Vial Problem
Every number above assumes the vial contains 10 mg of tirzepatide. That assumption does more work in a microdosing protocol than in a standard one, and it is the weakest link in the chain.
Lyophilised peptide powder is typically not 100 percent peptide by mass, and content varies between suppliers and between batches.
