Two Pathways Sold Under One Label
An erection is the end of a chain, and things marketed as peptides for erectile dysfunction intervene at very different points in it. Understanding which point a compound targets explains almost everything about whether it can plausibly help.
The peripheral half is the one most people know. Arousal signals travel down through the spinal cord, nitrergic nerves and endothelium release nitric oxide in the corpus cavernosum, nitric oxide raises cyclic GMP, cGMP relaxes smooth muscle, blood fills the sinusoids and venous outflow is compressed. Sildenafil and tadalafil work here by inhibiting the enzyme that degrades cGMP. They do not create arousal, they preserve the mechanical consequence of it, which is exactly why they fail when the upstream signal never arrives.
The central half is where peptides live. In the hypothalamus, particularly the paraventricular nucleus, melanocortin signalling at MC4R and dopaminergic and oxytocinergic projections shape sexual desire and the descending pro-erectile output. A melanocortin agonist reaching those receptors can initiate arousal signalling rather than amplify its downstream plumbing. That is a genuinely different mechanism, and it is the only reason peptides are in this conversation at all.
PT-141 (Bremelanotide): The Only One With an Approval
PT-141 is a cyclic heptapeptide derived from melanotan II, developed after researchers noticed that melanotan II trial participants were reporting spontaneous erections as a side effect of a tanning compound. Chemists stripped the molecule back toward the melanocortin activity responsible for the sexual effect and away from the pigmentation activity, which is the entire origin story of this compound.
It acts as a non-selective melanocortin receptor agonist with the relevant action at MC4R in the hypothalamus. Because the effect is central, it does not require intact endothelial nitric oxide signalling to initiate a response, and it is not dependent on the same vascular machinery a PDE5 inhibitor needs.
What the Trials Showed
The male erectile dysfunction programme ran first. Intranasal bremelanotide produced measurable erectile responses in men, including in a subgroup who had responded poorly to sildenafil, which was the most interesting finding to come out of the work. That programme was halted over blood pressure increases seen with the intranasal route at the doses being used.
Development then moved to subcutaneous dosing and to female hypoactive sexual desire disorder, where the RECONNECT trials led to approval in 2019 for premenopausal women at 1.75 mg subcutaneously as needed. That is the only approved indication anywhere. Men buying PT-141 for erectile function are using a compound with real supporting data for a use it was never licensed for.

Kisspeptin: Working Further Upstream
Kisspeptin sits above the entire reproductive axis. It signals through KISS1R on GnRH neurons and is the permissive switch for GnRH release, which drives LH and FSH, which drives testosterone. That places it a level above anything else discussed here.
Imperial College London ran a series of studies administering kisspeptin-54 to men, including men with hypoactive sexual desire disorder. Functional imaging showed enhanced activity in limbic structures in response to sexual stimuli, and the work reported behavioural and self-reported changes alongside the hormonal effects. The signal is interesting and the trials were well designed, but the numbers were small and the administration in most of the work was intravenous infusion in a research setting.
That is the honest position on kisspeptin: a legitimate research target with early human data, and no established protocol, dose or safety record for the way it is sold in the research chemical market. It is the compound most worth watching in this space and the one most oversold relative to what has been demonstrated.
Melanotan II: The Accident That Started This
Melanotan II is where the melanocortin and sexual function link was first noticed, and it does produce erectile responses in men, including in trials specifically examining that effect. It is also a blunt instrument. It activates MC1R (pigmentation), MC3R and MC5R alongside MC4R, which means anyone taking it for sexual function is also getting tanning, appetite suppression and whatever else broad melanocortin agonism produces.
The side effect load is why PT-141 exists at all. Nausea, flushing, spontaneous and unwanted erections, darkening of existing moles and the unresolved question of what indiscriminate melanocyte stimulation does over time all sit on the melanotan II side of the comparison. Compounds Sold for This With No Human Evidence
A large share of what is marketed as peptides for erectile dysfunction has nothing behind it for this indication. Being specific is more useful than a general warning.
- BPC-157. Rodent work reports protection against drug induced erectile disturbance and involvement of nitric oxide system modulation. No human trial exists for this use. The general evidence picture is in the BPC-157 guide.
- Growth hormone secretagogues. Ipamorelin, CJC-1295, sermorelin and MK-677 raise GH and IGF-1. Neither GH nor IGF-1 has a demonstrated role in treating erectile dysfunction, and the marketing here is a general vitality claim wearing a mechanism it does not have.
- Oxytocin. Involved in orgasm and post-coital physiology, with no demonstrated effect on erectile function. It is an affiliative compound, not an arousal compound, and the evidence for the intranasal route is weak in general. See the oxytocin peptide guide.
- Blends and proprietary sprays. Unlabelled combinations sold for sexual performance carry the dosing uncertainty of every component at once, and any effect they produce is usually attributable to a single known compound at an undisclosed amount.
The pattern is consistent across this category: a plausible mechanism from animal work gets presented as a clinical finding. The same gap between mechanism and demonstrated effect shows up whenever a compound is compared against pharmaceuticals, which is covered from the other direction in peptides versus steroids.
How Peptides Compare to PDE5 Inhibitors
PDE5 inhibitors are the reference standard for a reason. They have decades of safety data, response rates around 60 to 80 percent in general erectile dysfunction populations, oral dosing and a well characterised interaction profile. Any peptide is being measured against that.

The one situation where the central approach has an argument is a PDE5 non-responder whose vascular function is intact, or someone whose primary complaint is desire rather than rigidity. Combining a melanocortin agonist with a PDE5 inhibitor has been studied and is not a casual stack, because both influence blood pressure and the interaction has not been characterised for unsupervised use.
The other thing worth saying plainly: erectile dysfunction correlates strongly enough with cardiovascular disease that it is treated clinically as an early warning sign, often preceding a cardiac event by several years. A compound that produces a result without addressing that is removing a symptom that was carrying information.
Side Effects and Who Should Not Experiment
PT-141
- Nausea. The defining side effect, dose related, around 40 percent in trials, occasionally severe enough to stop use entirely.
- Flushing and headache. Common, generally mild, resolving within the dosing window.
- Transient blood pressure increase. A small rise in systolic and diastolic pressure with a compensatory heart rate reduction in the hours after dosing. This is the reason the intranasal male programme stopped.
- Injection site reactions and darkening of skin or gums. Pigmentation effects appear with repeated dosing even at the improved selectivity.
Hard Contraindications
- Uncontrolled hypertension or known cardiovascular disease. The pressor effect is the specific concern, and it is not theoretical.
- Concurrent nitrates. Relevant to anyone also using or considering PDE5 inhibitors, where the interaction is dangerous.
- Untreated underlying cause. Diabetes, hypogonadism, sleep apnea or medication effects should be diagnosed before anything is layered on top.

How long does PT-141 take to work?
The approved protocol has it dosed about 45 minutes ahead of anticipated activity, with the effect building over roughly one to two hours. It is slower to arrive than a PDE5 inhibitor and the window is measured in hours. Unlike tadalafil, it is not a compound that establishes a background state across days, so timing is part of using it at all.
Can PT-141 and sildenafil be combined?
Combination has been studied but it is not a casual stack. Both affect blood pressure through different mechanisms, PT-141 produces a transient pressor effect while PDE5 inhibitors are vasodilators, and the net cardiovascular result in an unsupervised setting is not well characterised.
