Rybelsus vs Ozempic: Semaglutide Tablet vs Injection

The short answer

Rybelsus and Ozempic both contain semaglutide from Novo Nordisk and both are labelled for type 2 diabetes. Rybelsus is a daily tablet (3, 7 or 14 mg) taken on an empty stomach; Ozempic is a weekly injection (0.25 to 2 mg). The January 2026 label also adds Ozempic tablets. The forms are not substitutable milligram for milligram.

Rybelsus and Ozempic both contain semaglutide from Novo Nordisk. Rybelsus is a tablet taken once a day. Ozempic is best known as a once-weekly injection under the skin. The picture changed with the current US label, revised January 2026, which covers Rybelsus tablets and a second tablet product, Ozempic tablets, in one document. This page lines up the injection and the tablets from Novo Nordisk's labels. It is a research reference, not medical advice.

The tablets and the injection are not interchangeable on a milligram basis. The Rybelsus label gives a switching rule: patients on Ozempic 0.5 mg injection can move to Rybelsus by starting 7 mg or 14 mg once daily one week after the last injection.

Ozempic tablets: the third product

The January 2026 label describes Ozempic tablets in 1.5 mg, 4 mg and 9 mg strengths. They follow the same pattern as Rybelsus (30 days at the lowest strength, then the next step on day 31, then the top strength from day 61 if needed): 1.5 mg, then 4 mg, then 9 mg. The label says Rybelsus and Ozempic tablets are not substitutable mg for mg, and that after the first 30 days a patient can switch between them: 7 mg of Rybelsus matches 4 mg of Ozempic tablets, and 14 mg matches 9 mg. This is the reason a search for "Ozempic" can now mean a tablet, not only the pen.

What the label trials show

The two products have no head-to-head trial in the labels, and their trials ran for different lengths, so the table below shows what each label reports, not a ranking.

TrialDoseHbA1c changePlacebo HbA1c changeWeight change (placebo)
Rybelsus, 26 weeks, 703 adults (label)7 mg, 14 mg daily-1.2, -1.4 points-0.3 points-2.3 kg, -3.7 kg (-1.4 kg)
Ozempic injection, 30 weeks, 388 adults (label)0.5 mg, 1 mg weekly-1.4, -1.6 points-0.1 points-3.8 kg, -4.7 kg (-1.2 kg)

Both trials enrolled adults with type 2 diabetes who were inadequately controlled on diet and exercise, with mean baseline HbA1c around 8%. The tablets lowered HbA1c and weight, but by less than the injection did in its own trial. A 26-week and a 30-week trial are different lengths, so treat the gap as a pointer, not a finding.

Side effects on the two labels

Nausea11%, 20% (6%)15.8%, 20.3% (6.1%)
Abdominal pain10%, 11% (4%)7.3%, 5.7% (4.6%)

Vomiting6%, 8% (3%)5.0%, 9.2% (2.3%)
Constipation6%, 5% (2%)5.0%, 3.1% (1.5%)

Most nausea, vomiting and diarrhea came during dose escalation on both. In the pooled tablet trials, 8% on 14 mg and 4% on 7 mg stopped because of gastrointestinal effects, against 1% on placebo; in the Ozempic pool it was 3.1% on 0.5 mg and 3.8% on 1 mg, against 0.4% on placebo. The two labels pool different trials.

Which is for weight loss?

Neither label lists weight loss as an indication. For weight, the semaglutide brands are Wegovy (injection and a tablet) and the weight-loss use of oral semaglutide is covered separately in oral semaglutide for weight loss. The injection pair is explained in Ozempic vs Wegovy.

Research-grade semaglutide and the calculator

Compounded or research-grade semaglutide is a different category from any of these brand products; see compounded semaglutide. If you work from an injectable vial, the calculator turns vial strength, water volume and dose into a syringe mark. It does not apply to tablets.

Frequently asked questions

Is Rybelsus the same as Ozempic?

Both contain semaglutide from Novo Nordisk, but they are different products: Rybelsus is a once-daily tablet and Ozempic is a once-weekly injection. The current label also covers Ozempic tablets, so "Ozempic" can now mean a tablet or a pen.

Is Rybelsus as effective as Ozempic?

There is no head-to-head trial in the labels. In their own monotherapy trials, Rybelsus 7 mg and 14 mg lowered HbA1c by 1.2 and 1.4 points over 26 weeks and Ozempic 0.5 mg and 1 mg by 1.4 and 1.6 points over 30 weeks. The trials ran for different lengths, so the gap is a pointer, not a ranking.

How do you take Rybelsus?

Once daily in the morning on an empty stomach, swallowed whole with no more than 4 ounces of plain water, then wait at least 30 minutes before eating, drinking anything else or taking other oral medicines.

Can you switch from Ozempic injections to Rybelsus?

The label says patients taking Ozempic 0.5 mg can switch to Rybelsus by starting 7 mg or 14 mg once daily one week after the last injection. The decision belongs to a prescriber.
