Does Zepbound Need to Be Refrigerated? How Long Tirzepatide Lasts Out of the Fridge
Yes, Zepbound is stored in the refrigerator. The more useful question, and the one people are usually asking, is how long it survives outside one: the pen left on the counter overnight, the vial in a hotel room, the week away from home. The label answers that with a number, and the number is more generous than most people assume. This page gives the label figures for Zepbound, Mounjaro, Wegovy and Ozempic, then covers compounded and research vials, which have no label and follow different rules.
The short answer
Zepbound pens and vials live in the fridge at 36 to 46 F (2 to 8 C). Once out, the label allows up to 21 days at room temperature not above 86 F (30 C). Never frozen, out of direct light, back in the fridge is fine within those 21 days. Compounded tirzepatide has no label allowance: keep it cold and follow the pharmacy's beyond-use date. A reconstituted research vial should be treated as a fridge-only item.
The label figures, side by side
These are the storage statements from the US prescribing information for each product, as of October 2026. Check the leaflet in your own box, because labels are updated.
| Product | Before first use | Out of the fridge | Never |
|---|---|---|---|
| Zepbound (tirzepatide) pen or vial | Refrigerate, 36 to 46 F, in the carton | Up to 21 days at or below 86 F (30 C) | Freeze. Discard if it has frozen. |
| Mounjaro (tirzepatide) pen or vial | Refrigerate, 36 to 46 F, in the carton | Up to 21 days at or below 86 F (30 C) | Freeze. Discard if it has frozen. |
| Wegovy (semaglutide) pen | Refrigerate, 36 to 46 F | Up to 28 days at 46 to 86 F (8 to 30 C) | Freeze. |
| Ozempic (semaglutide) pen | Refrigerate, 36 to 46 F | After first use, 56 days refrigerated or at 59 to 86 F (15 to 30 C) | Freeze. Keep the cap on, away from light. |
| Compounded tirzepatide or semaglutide vial | Refrigerate; follow the pharmacy label | No label allowance. Ask the pharmacy; many say days, not weeks | Freeze. Use beyond the beyond-use date. |
| Research vial, reconstituted | Refrigerate, use within 2 to 4 weeks | Not a storage option. Hours for an injection, not days | Freeze or leave in light. |
What the 21 days actually means
The allowance is cumulative time out of the fridge, not a fresh 21 days each time the pen comes out. A pen that spent a week on a trip and goes back in the fridge has 14 days of room temperature left in it. It is also a ceiling, not a target: the drug does not fail on day 22, but Lilly tested stability to that point and no further, and the product is priced too high to test the rest yourself.
The 86 F limit matters more than the day count in practice. A parked car in summer passes 86 F in minutes. A checked suitcase in a hold can go either way. A kitchen counter in a normal home does not come close. If a pen has been somewhere hot, the label's instruction is to discard it, and a pharmacist will say the same.
Does it need to be cold when you inject it?
No, and it is better if it is not. Cold liquid stings more going in. Take the pen or vial out 20 to 30 minutes before the injection and let it reach room temperature. That half hour counts against the 21 days in theory and makes no practical difference. Technique is in the injection guide; the dosing schedule itself is in the Zepbound guide and the tirzepatide dosage guide.
Compounded tirzepatide is a different case
A compounded vial from a 503A or 503B pharmacy has no manufacturer stability study behind it. Its storage rules are the pharmacy's beyond-use date and its own label, which usually says refrigerate and gives a short window at room temperature, if any. Some pharmacies say up to a few days; some say none. Treat the pharmacy's figure as the answer and the branded 21 days as irrelevant to your vial. More on sourcing and what to ask the pharmacy in compounded tirzepatide.
Research-grade tirzepatide bought as lyophilised powder follows the general rules in the peptide storage guide: powder is tolerant and lasts months cold and dry; once reconstituted with bacteriostatic water it is a two to four week item in the fridge, and room temperature is for the minutes around an injection, not for storage.
Travelling with it
For most trips the label does the work: 21 days at room temperature covers a holiday, and a hotel fridge covers the rest. Carry it in your hand luggage, never the hold, and keep it out of the sun and the car. Gel packs are allowed through US security for medically necessary items and may be partially frozen; the rule text is in the storage case guide.
Where a cooler earns its place is heat and length: a summer road trip, a destination above 86 F, a hotel with no fridge, or vials rather than pens. The peptide fridge guide covers what each type of cooler actually holds. Of the small battery units, the VialHaus Cold is built for vials standing upright and states the room temperature it works in, which most do not (disclosure: same founder as the Bureau).
Signs a pen or vial should be discarded
- It has been frozen, even once.
- It has spent time above 86 F, or more than 21 days in total out of the fridge.
- The liquid is cloudy, coloured or has particles in it. Tirzepatide and semaglutide are clear and colourless.
- It is past the expiry date on the carton.
None of these make the drug dangerous in the dramatic sense; they make it unreliable, and an unreliable dose at $1,000 a box is the expensive kind of mistake.

Frequently asked questions
Does Zepbound need to be refrigerated?
Yes. Store it at 36 to 46 F in the original carton. The label then allows up to 21 days out of the fridge at or below 86 F.
How long can Zepbound be out of the fridge?
Up to 21 days in total at room temperature not above 86 F (30 C). The days add up across trips; they do not reset when the pen goes back in the fridge.
How long does tirzepatide last in the fridge?
Branded Zepbound and Mounjaro last until the expiry date on the carton if kept at 36 to 46 F and unfrozen. A reconstituted research vial is usually treated as good for two to four weeks refrigerated.
Does tirzepatide need to be refrigerated?
Yes by default, for every form. Branded products have a 21-day room temperature allowance; compounded vials follow the pharmacy's label, which is usually shorter; research vials have none once mixed.
How long can compounded tirzepatide be out of the fridge?
Only as long as the pharmacy that made it says. There is no manufacturer stability data behind a compounded vial, so the branded 21 days does not apply. If the label is silent, ask the pharmacy and assume hours rather than days.
Can I put Zepbound back in the fridge after it has been out?
Yes, as long as it has not been frozen or above 86 F, and the total time out stays under 21 days.
